Community Matron Service
Community Matrons provide advanced clinical assessment and intensive case management (co-ordination of care) for patients who have more than one long term condition such as Diabetes, Coronary Heart Disease, Arthritis and many more conditions.
Community Matrons also support the families of people with long term conditions and often work closely with hospital consultants, GPs, other nursing staff and social workers to ensure people get the help they need.
Who can use the service?
The service is offered to all patients registered with a Luton GP who are aged 18 and over and meet 2 or more of the following criteria:
- recent exacerbation or deterioration of chronic illness (within past 90 days)
- more than 2 falls within previous two months
- recently bereaved and at risk of medical decline
- cognitively impaired and medically unstable
- taking 8 or more medications
- 2 or more hospital admissions within a 12 month period.
The service is provided from:
The service is based at The Poynt, Units 2-3, Poynters Road, Luton, LU4 0LA. The service is delivered in the patient’s place of residence, for example home, residential or care home.
Services provided include:
The Community Matron Service incorporates a team of experienced nurses who support patients with long term conditions, closely monitoring their well-being in order to prevent unnecessary hospital admission.
The nurses work autonomously with advanced clinical skills and prescribe for their patients if necessary, making appropriate referrals when required, acting the coordinator of care for all health professionals involved in the patients care.
The service is provided 7 days a week, 24 hours a day working in close conjunction with the Out of Hours Community Nursing team to provide cover during the evening and nights.
Patients are encouraged to call their community matron in the first instance as soon if they feel unwell or there is a change in their health status.

District Nursing
Community nursing services include district nurses and community matrons. District nursing teams assess, plan and provide nursing clinical care to those people who are often housebound due to ill health, either in their own home, or in a care home that does not provide nursing.
They also support:
- End of life care
- post operative care
- management of long term conditions
- wound care.

The Community Nursing Service is provided within patient’s own homes including residential care homes. The Community Nursing Service is based at The Poynt, Units 2-3, Poynters Road, Luton, LU4 0LA. It is available 24 hours per day, 365 days a year.

Service provided include:
The Community Nursing Service provides routine and complex, scheduled and unscheduled care working collaboratively with GPs, social services, and a range of other hospital and community health services.
Examples of care provided include:
- Routine Care: such as diabetes care, post operative care, simple/complex wound care, leg ulcer management, catheterization, bowel management and management of patients requiring enteral feeding.
- Complex care including end of life care to patients with cancer and palliative care needs,
- Scheduled Care i.e. planned interventions as identified in the patient’s care package
- Unscheduled Care i.e. intervention(s) required in addition to the identified care package for example catheter blockage in the out of hours period, administration of medicines including intravenous/central line therapy, syringe driver and injection.
