Heart Failure Service - Luton

The community heart failure service provides support and monitoring for patients with a diagnosis of left ventricular systolic dysfunction.

Patients living in Luton or registered with a Luton GP.

Service provided include:
Our service works closely with the Luton and Dunstable Hospital nurse led heart failure clinic to identify patients that have been admitted with heart failure and would benefit from a community based service. 

The two services work closely together to improve the patient’s experience and outcomes.  

The service offers: home visits, clinic appointments and a heart failure rehabilitation programme.  The service also seeks to promote self management and care through patient education and support, whilst sign-posting patients to other agencies and services where available. 

Training programmes on the management of heart failure are also delivered to health care professionals  to improve knowledge and promote best practice.  
  
Referrals to this service can be made by:
Access to the service is gained by referrals from GPs, the hospital based heart failure nurse specialist or other health care professional.
