Specialist support

Talk to community respiratory, heart failure, and cancer teams for support from specialist nurses. Get in touch on 0333 405 3000.

Rapid response

For acute health assessment and monitoring if you're worried about a patient today, call rapid response on 0333 405 3000 and they will escalate as needed. Find out more about the service here.

For falls prevention advice for those at risk of falling, and those who have already fallen. Find out more about the team here. 

Frailty step up bed 

For an acute health need, access to a short term bed for your patient via the rapid response team, on 0333 405 3000.

• Talk to your At Home First co-ordinator if you would like to raise a patient over 65 to the daily huddle or weekly MDT for discussion.
