The Service is a team of experienced nurses able to provide acute (short term) medical and nursing care in patients own homes, residential care homes for the elderly or an intermediate care setting to prevent hospital admissions or facilitate early discharge.
Service provided include:
- The team provides assessment, support and nursing care to patients in their own homes including care homes and if required for those needing a period of intermediate care/ rehabilitation beds.
- The team provide a first point of contact for advice and nursing assessment for acute conditions to people living in residential care homes in Luton.
- The team consists of skilled nurses who are trained to undertake full assessment of patients, deliver acute interventions such as intravenous drug therapy and monitor patients during acute episodes of illness.
- The team work collaboratively with GPs, Consultants and other services such as the Community Assessment and Rehabilitation Team and social services.

The service is provided to adults aged 18 and over and registered with a Luton GP.
Referrals to this service can be made by:
- GP’s including Out of hours GP Services
- Hospital Consultants / Hospital Clinical Navigation Team
- Social workers
- staff working in residential care homes for the elderly in Luton
- Ambulance personnel
