Our liaison team work with other organisations as part of the integrated discharge team based at Luton and Dunstable Hospital.
The team have the lead for admission avoidance in the role of GP liaison by discussing with referrers patients who may require a same day assessment by the acute trust. The team advise and suggest possible alternatives to a hospital attendance and also have a role in identifying and developing patient pathways and community services.
The team also have responsibility for facilitating complex discharges from the L&D hospital;
- Assessing and prescribing of equipment
- Facilitate and support complex discharges with health funding
- Referrals to community services
- Advice and support to hospital staff/patients/carers re community services
- Liaison with voluntary and statutory services
