Our adult services joined forces with Luton GPs to trail a new model of care which aimed to significantly improve the quality of care and patients' experiences. The Primary Care Home Model pilot project. launched by the National Association of Primary Care (NAPC) was initially tested in 15 locations across the country. 

Our teams completed a three month pilot with the Medics United GP Cluster testing a Polypharmacy pathway to improve the care of patients over the age of 75 taking more than 10 medications. And they worked with the Kingsway GP Cluster to test a pathway of working with diabetic patients, particularly of South East Asian origin, who were failing to engage with health professionals.
