Accessing your medical records
You have a right to request copies of your medical records. This is known as a ‘subject access request’. We will provide you with copies of your records. It may take 1 month for us to process your request.
We will remove any information that you do not have the right to access. This is also known as redaction. Any information removed will be in-line with the Data Protection Act 2018 exemptions.

You can access someone else's records, if you have proof that you have the right to access them.
You can access the records of someone who died. We process these the same way as a typical subject access request. A subject access request for someone who died is governed by the Access to Health Records Act 1990.
Cambridgeshire Community Services NHS Trust (CCS) records
We can process subject access requests for people who have used our services.
We will need:
- proof of your identity - this can be a copy of your passport, birth certificate or driver's licence
- proof of your address - this can be a copy of a bill (for example council tax bill or utility bill) sent to your address in the last 6 months
- proof you have a right to request someone else's records (if applicable) - this can be a copy of a child's birth certificate, a letter showing power of attorney has been granted or their signed consent.
If possible, try to include your NHS number and types of services you've received from us. This will help us to process your request.
We will start processing your request once we have enough information. We have a month, from the date we have enough information, to process your request.
The information you provide will only be use to process your request and track our performance. Copies of your request will be kept for 3 years.
Once we have your records, we will send them to you via secure email.
