Children's continuing care nursing service

What we do

This service provides a community based children’s nursing service for children with very complex health needs to help them achieve their full potential. Our service is provided for children aged 0-18 years’old and registered with a Luton GP and their families.

Nursing care is provided 365 days a year, seven days a week in a range of community settings, including the home, social and educational.

Services provided include:

  • The aim of the service is to improve the quality of life of any child with complex needs and his/her family  
  • A personalised care plan is developed for every child in collaboration with their parent/ carer providing care at home wherever possible, and support, education and relevant training to parents as requested. 
  • The team will also make referrals for additional care packages or signpost families to other services and agencies available and provide education and training to other professionals involved in the child’s care.
