Service provided include:
Our Specialist palliative care Clinical Nurse Specialist service is a 7 day service that is  part of an integrated model, working closely with all health and social care services in Luton. 

Our service focuses on:

  • Providing expertise in symptom management including pain management and psychological needs. We support and help patients and their families plan for the future, by helping patients and their loved ones to think about and record what matters to them in their future care.
  • Offer psychological support to help adjust to living with a life limiting illness
  • Promote living well and dying well in the identified preferred place of care
  • Offer family support and signpost to additional help when required
  • The Macmillan palliative care team is made up of specialist palliative care clinical nurse specialists and palliative care support workers supported by a palliative care consultant. 

The service is also supported by a consultant psychologist, family therapist and a welfare benefits advisor (where appropriate). The team works closely with patients GPs, consultants and Keech hospice.

Who can use the service?
Adults registered with a GP in Luton who are living with any life limiting condition.It is particularly important for people who require end of life care or have advanced disease.

Referrals to this service can be made by: 

  • Any health/ social care professional for patients, families and carers

Clinical care is delivered in conjunction with the Luton and Dunstable Hospital, Keech Hospice, GPs and other professionals:

  • In the patient’s own homes including care homes
  • Community based clinics.
