Norfolk Healthy Child Programme 5-19 Pathway (School Nursing)

The team provides a service to all Norfolk children of statutory school age and their families and/or carers. We work with children and young people attending mainstream schools, complex needs schools, short stay schools or out of education. Contact can be arranged in a variety of settings, including school, a health clinic or at home.

The team is comprised of Specialist Community Public Health Nurses (SCPHN - School Nurses), Family Public Health Nurses (Dual qualified School Nurse & Health Visitor), Community Staff Nurses, Resilience and Healthy Lifestyles Practitioners, Assistant Practitioners and Support Workers. This service follows on from the services provided by our 0-5 colleagues (Health Visiting).

Our aim is to work with children, young people and their families to enable them to play an active role in making healthy life choices. We want to support children and young people to reach their full potential through offering support and advice in the following areas:-

  • Emotional wellbeing
  • Sexual health and healthy relationships
  • Healthy Lifestyles eg sleep, smoking cessation, alcohol and drug misuse, healthy eating
  • Transition on starting school
  • Moving to high school and leaving school
  • Health issues impacting on school attendance
  • Health literacy – advice on accessing other health services and resources
  • Hearing and vision screening (Reception age)
  • Requested hearing tests
  • National Childhood Measurement Programme (Reception and Year 6)
  • Text messaging Service for young people – Chat Health ( 07480635060)
  • Safeguarding vulnerable children and young people, working closely with other agencies

How to get in touch with us

  • All our services including telephone consultation can now be accessed through our single point of access - Just One Number(JON) - 0300 300 0123
  • In writing - a referral form is available Preferred method is for referral to be e mailed to ccs.nhcp-norfolkcyphealthservices@nhs.net
  • Referral forms are available through JON and from schools
  • Referrals are accepted from young people, parents/carers and professionals
  • The Duty office can be contacted for further advice on making a referral.
  • Interpretation and translation services can be arranged by staff, for users of this service.

ChatHealth: a confidential text service for young people in Norfolk Poster for Chathealth

Do you need confidential advice and support from a School Nurse? Then contact our text messaging service for 13-19 year olds on 07480 635060.

ChatHealth is an easy way for young people to confidentially seek help about a range of issues, make an appointment with a school nurse, or find out how to access other local services including emotional support or sexual health services.

Messages sent to the dedicated number are delivered to a secure website, and responded to by a school nurse. Out of hours, anyone who texts the service will receive a bounce back message explaining where to get help if their question is urgent, and when they can expect a response. Texts are usually replied to within one working day.
