Access to Medical Records

Requesting Your Information

You have a right to request copies of your medical records. This is known as a ‘subject access request’. We will provide you with copies of your records, with any information which you do not have a right of access to removed (redacted). This will be done in line with the exemptions to subject access which are set out in the Data Protection Act 2018. You can find out what these are in our Subject Rights Request Policy

The records of a deceased person can also be accessed and these are processed in the same way. These requests are governed by the Access to Health Records Act 1990.

To make your request you can write to us or send an email to ccs.accesstoinfo@nhs.net. We will need to see proof of your identity, a copy of your passport, birth certificate or driver's licence and proof of your address, a utility bill, council tax bill or similar, sent to your address in the last 6 months. Your NHS number and an indication of the service you received from us will help us process your request.

If you are asking for access to someone else’s records, we will need to see proof that you have a right to make that request on their behalf e.g. a copy of a child’s birth certificate or a letter showing power of attorney has been granted or their signed consent, where they are able to give this.

You can also use the Subject Access Application Form to make your request. 

We have a month to process your request, from the date we have sufficient information to process it. The information you provide will only be used to process your request and monitor our performance. Copies of the request will be kept for 3 years.

Once we are in receipt of your records we will dispatch them to you via Royal Mail recorded delivery.
